Purpose: To report two rare cases of the double-layered anterior capsule during the cataract surgery.
Case summary: A 67-year-old female patient who presented with bilateral cataract underwent phacoemulsification and intraocular lens implantation. Continuous curvilinear capsulorhexis was performed, and double-ring sign and radial tear of lens capsule were observed during phacoemulsification. The intraocular lens was implanted in the capsular bag and no specific findings were observed. A 76-year-old man underwent cataract extraction and intraocular lens implantation in his right eye. Double-ring sign was observed when phacoemulsification and intraocular lens implantation were performed after continuous curvilinear capsulorhexis. The surgical procedure proceeded without any specific findings.
Conclusions: A double-ring sign was observed in patients who had no previous history of trauma, uveitis, or thermal exposure and who had no abnormal findings in the anterior capsular lens before surgery. The authors report two cases of the double-layered anterior capsule during cataract surgery which may be different to typical true exfoliation.
